Our work on transformation projects is focusing on three broad areas:
Integrated Care System: Maximising the opportunities offered by the Berkshire West Integrated Care System to develop new models of care.
Access: Developing joint-working between practices to ensure appropriate patient access to the right healthcare professional at the right time.
Alternatives to Outpatients: Exploring how patients can be seen locally by specialist services when previously they would have had to have travelled to hospital.
